FEATURED SEARCH TERM: PSA after surgery
Biochemical control after radical prostatectomy in high-risk prostate cancer patients has proven disappointing, even when paired with low-dose external-beam radiation therapy (EBRT). However, high radiation doses have demonstrated improved biochemical and clinical results. Radiation oncologists in Spain adopted the combination of EBRT and high-dose-rate (HDR) intensity modulated brachytherapy as their standard treatment protocol for patients with prostate-specific antigen (PSA) levels of more than >20 ng/ml. In a long-term outcomes report, they outline the advantages that EBRT with an HDR boost offered over conventional surgery, including a longer time to PSA failure.

Despite supportive data for adjuvant radiation therapy after radical prostatectomy for high-risk prostate cancer, many clinicians are still cautious about salvage radiotherapy. A recent survey queried radiation oncology and urologists on when and why they recommend radiation therapy after radical prostatectomy. Not surprisingly, their responses differed by their specialty, their understanding of radiotherapy, and their concerns about treatment-related toxicity.
